  • Inform the provider of their physical location when requested.

  • Provide emergency contact information when requested.

  • Inform the provider of changes in symptoms, medications, or medical conditions.

Telehealth appointments with Rise Psychiatry Physician Assistant Corporation are not emergency services.

If an emergency occurs, emergency medical services or other appropriate local resources may be contacted when clinically and legally appropriate.
